Together, we are committed to building healthier communities by delivering comprehensive, quality health care regardless of socioeconomic status. From the moment a patient walks through our doors, they are treated with one-on-one care in a professional, dignified and compassionate manner.

Quick Onboarding of New Patients

We understand that new patients don’t need to see the inside of a waiting room—they need to see a care provider. That’s why we pride ourselves on quickly getting new patients set up with the information and care they need so they can move on with their lives.

Affordable Care & Sliding Payment Scale

We provide affordable, high quality care to residents across northeast Indiana. A sliding fee discount is available to those who meet specific income guidelines and may not have met their insurance deductible, whose insurance does not cover the service provided, or are uninsured. Transportation Assistance

We work with many commercial insurance plans, such as CHIP, Medicaid, Medicare, and Marketplace.

Insurance Enrollment Services

This FREE service to the community and our patients helps residents navigate the application process, submit documents, maintain coverage, and help to answer any questions about coverage plans.

Integrated Services

We offer medical, dental, vision, behavioral health, and WIC services. For the convenience of our patients, these are provided in a centralized location for easier transition between services to provide care for the whole person.
